The “smart welfare” project is granted by Lombardy Region with the aim to elaborate a social chart shared by all the Municipality of Monza e Brianza Province (55 municipality).
Infact in Lombardy the Municipalities have the task to face most of social problems of their citizens and to collaborate with health workers for those disease that involves social difficulties (i.e. lonely elderly)
The “smart welfare” project foresee the creation of a digitized social chart managed by the social worker of the municipality where the patient lives.
The social patient chart contains all the information needed to plan and manage the social project developed to improve the situation of the citizen and her\his family.
The social worker will add all the information time by time, but several data are added thanks to smart links
In fact the social chart can automatically gather data from civil registry, health offices (i.e. legal disability; paediatrician or general practitioner; elderly specific therapies, juvenile court acts; etc.), schools; patrimonial and economic governmental agencies; so that you can have right away the whole picture of the situation of the family involved in the social project.
Taking into account that the social digitized chart is shared with the 55 town of the district every social or medical worker involved has the same and complete information at the same time.
As the citizens of the district are about 862684 it will be possible to analyze the data and have a whole idea of the problems in social and health areas. In this way the public administrators will be able to elaborate a more appropriate and update answers to the citizens’ needs.
Further, it is now easy to answer quickly and without additional work to the administrative periodical surveys from many authorities (such as Region).
The aim is to reduce more and more the sheet charts, add other smart links and make all the inner decisional process taken via pc.
Also Umbria Region adopted this instrument and other Italian regions are interested in it so far.
