The “smart welfare” project is granted by Lombardy Region with the aim to elaborate a social chart that can be used by the social workers of all the Municipality of Monza e Brianza Province (55 municipality).
The social patient chart contains all the information needed to plan and manage the social project developed to improve the situation of the citizen and her\his family.
In fact the social chart can automatically gather data via smart links from civil registry, health offices (i.e. legal disability; paediatrician or general practitioner; elderly specific therapies, juvenile court acts; etc.), schools; patrimonial and economic governmental agencies; so that the social worker(s) have all the informations needed to understand the situation of the family involved in the social project.
Taking into account that the social digitized chart is shared with the 55 towns of the district, every social or medical worker involved is updated about the situation
Public authorities will be able to access all the data gathered in the social chart, analyse the data and have a comprehensive overview of health and social issues in order to respond with appropriate policy measures to the different kind of citizens’ needs.
Further, it is now easy to answer quickly and without additional work to the administrative periodical surveys from many authorities.
This tool has an important benefit in terms of resource efficiency, thanks to the smart share of information and the digital process. Moreover, it contributes to reducing raw material because paper and ink are saved. For example, thanks to smart welfare project, it is not necessary any moreto send copies of documents to the several offices.
Also the Umbria Region has adopted this instrument and many other Italian regions showed interest.
